Twynsta (Amlodipine/Telmisartan)

Do’s

• Before starting Twynsta or each time you get a refill, read the Patient Information Leaflet provided by your pharmacist. If you have any questions, ask your healthcare provider.
• Swallow the medication by mouth with or without food, as directed by your healthcare provider. It is typically taken once daily.
• The dosage of Twynsta will be determined by your healthcare provider based on your specific medical condition and individual response to treatment. Follow the prescribed dosage.
• To get the most benefit from Twynsta, use it regularly. Taking it at the same time each day can help you remember. 
• Even if you start feeling well, continue taking the medication as prescribed, as most people with high blood pressure do not experience noticeable symptoms.
• It may take up to 4 weeks before you start experiencing the full benefit of Twynsta. If your condition does not improve or if your blood pressure readings remain high or increase, inform your healthcare provider.

Don’ts

• Do not exceed or decrease the prescribed dosage without consulting your healthcare provider.

• Muscle pain, tenderness, or weakness
• Low sodium levels in the blood
• Swelling
• Dizziness
• Headache
• Fatigue
• Nausea
• Abdominal pain
• Back pain
• Respiratory tract infection

Twynsta contains amlodipine, which is a calcium channel blocker. In general, calcium channel blockers should not be used by individuals with congestive heart failure. While amlodipine does not seem to cause problems in people with heart failure, your healthcare provider may choose to monitor you more closely if you have this condition.

In rare cases, Twynsta may worsen chest pain or increase the risk of a heart attack in individuals with heart disease. This reaction is not common but is more likely to occur when the medication is first started or the dose is increased.

Twynsta can sometimes lower blood pressure too much, especially in individuals who are on a low-salt diet, taking diuretics (water pills) or are dehydrated.

In rare cases, Twynsta can cause a condition that leads to the breakdown of skeletal muscle tissue, resulting in kidney failure. If you experience unexplained muscle pain, tenderness, weakness, fever, unusual tiredness, or dark-colored urine, contact your healthcare provider immediately.

Twynsta should be used with caution in patients with mild to moderate hepatic impairment. The dosage of telmisartan should not exceed 40 mg once daily in these patients. Twynsta is contraindicated in patients with severe hepatic impairment.

Inform your healthcare provider if you are pregnant or breastfeeding before using Twynsta. Discuss the potential risks and benefits with your healthcare provider.
