Monopril HCTZ is a combination medication that contains two active ingredients: fosinopril sodium and hydrochlorothiazide (HCTZ). For example, antacids containing aluminum or magnesium can decrease the absorption of fosinopril. If you are taking such antacids, take them at least 2 hours before or after taking Monopril HCTZ. Similarly, if you are taking bile acid-binding resins (cholestyramine or colestipol) to lower cholesterol, take Monopril HCTZ at least 4 hours before or 4 to 6 hours after these medications.
  • To get the most benefit from Monopril HCTZ, take it regularly and at the same time each day. This will help you remember to take the medication consistently.
  • It may take several weeks before the full benefits of Monopril HCTZ are realized. Continue taking the medication even if you feel well, as most people with high blood pressure do not experience noticeable symptoms. 
  • If your condition does not improve or worsens, such as if your blood pressure readings remain high or increase, inform your healthcare provider.

  • Signs of high potassium levels (such as muscle weakness or irregular heartbeat)
  • Signs of low blood pressure (such as lightheadedness or fainting)

Monopril HCTZ can affect kidney and liver function. Regular monitoring of kidney function, including blood tests, may be necessary.

Monopril HCTZ can cause electrolyte imbalances, such as changes in serum electrolyte levels. Regular monitoring of electrolyte levels, including potassium, is important, especially if you are taking other medications that can affect electrolyte balance.

Monopril HCTZ can cause low blood pressure, especially when starting the medication or increasing the dosage. If you experience symptoms such as dizziness or fainting, contact your healthcare provider.
