Please fill out and sign the form, then send it to DoctorSolve +1-877-251-1650 (Fax Toll Free) or +1-604-595-8954 (International Fax) along with a copy of your initial prescription(s). DoctorSolve will authenticate your prescription with your doctor’s office. Additionally, we request a duplicate of any of the following documents: driver’s license, passport, military ID, or birth certificate.

1. To refill prescriptions online, go to Order a Refill and log in. You can also mail or fax your Refill Order Form
2. From your order history, select the medication that you would like to refill.
