Aventyl (Nortriptyline) is used to treat depression. It may help improve mood, relieve anxiety and tension, increase energy levels, and alleviate symptoms of depression.

Nortriptyline belongs to a class of drugs known as tricyclic antidepressants. Nortriptyline works by helping to restore the balance of certain natural substances in the brain, such as serotonin and norepinephrine.

Aventyl is also used off-label for conditions such as chronic pain, diabetic neuropathy, myofascial pain, orofacial pain, and postherpetic neuralgia.

Do’s

  • Take Aventyl exactly as prescribed by your healthcare provider, typically 1 to 4 times a day.
  • Use the provided measuring device for the liquid form, not a household spoon.
  • Follow your healthcare provider's instructions on dosage, especially regarding starting low and gradually increasing to minimize side effects.

  • Be patient with the medication - it may take up to 4 weeks to feel the full effect.
  • Continue taking Aventyl even if you feel well. Don't stop abruptly.
  • Tell your healthcare provider if your depression worsens or you experience suicidal thoughts.

Don'ts

  • Don't increase your Aventyl dose or take it more often than prescribed.
  • Don't stop taking Aventyl without consulting your healthcare provider, as withdrawal symptoms can occur.
  • Don't use a household spoon to measure the liquid medication.

Common side effects of Aventyl may include:

  • Drowsiness
  • Dizziness
  • Dry mouth
  • Constipation
  • Blurred vision
  • Headache
  • Nausea
  • Increased sweating
  • Weight changes
  • Trouble sleeping
  • Changes in taste

Serious side effects of Aventyl are rare but may include:

  • Allergic reactions such as rash, itching, swelling (especially of the face, tongue, or throat), severe dizziness, or trouble breathing.
  • Fast or slow heart rate
  • Palpitations
  • Fainting.
  • Confusion
  • Delirium
  • Seizures in some individuals
  • Eye pain
  • Swelling
  • Redness
  • Vision changes

Inform your healthcare provider about any allergies to nortriptyline, other tricyclic antidepressants, or any other medications before using Aventyl.

Tell your healthcare provider about any pre-existing conditions, especially:

  • Breathing problems
  • Liver problems
  • Recent heart attack
  • Difficulty urinating (enlarged prostate)
  • Overactive thyroid (hyperthyroidism)
  • Glaucoma (personal or family history, especially angle-closure type)
  • Mental/mood conditions (personal or family history, such as bipolar disorder, psychosis)
  • Family history of suicide
  • Seizures or conditions increasing seizure risk (brain disease, alcohol withdrawal)

Aventyl may cause abnormal heart rhythm (QT prolongation). This can rarely lead to serious, fast/irregular heartbeat, dizziness, or fainting requiring immediate medical attention.

The risk of QT prolongation increases with certain medical conditions or medications. Tell your healthcare provider about:

  • Specific heart problems (heart failure, slow heartbeat, Brugada syndrome)
  • Family history of heart problems (QT prolongation in EKG, sudden cardiac death)
  • Other medications you take
  • Low blood potassium or magnesium levels

Aventyl may cause dizziness, drowsiness, or blurred vision. Avoid driving, operating machinery, or activities requiring alertness until these effects subside. Limit alcohol and marijuana use which can worsen these effects.

Inform your healthcare provider about all medications you take (including Aventyl) before undergoing surgery.

Aventyl may increase sun sensitivity. Limit sun exposure, avoid tanning booths/sunlamps, use sunscreen, and wear protective clothing outdoors. Tell your healthcare provider immediately if you experience sunburn, skin blisters, or redness.

The liquid form of Aventyl may contain alcohol. Use caution if you have diabetes, alcohol dependence, liver disease, or need to limit/avoid alcohol. Consult your healthcare provider for safe use.

Aventyl may affect blood sugar control. Monitor your blood sugar levels regularly and inform your healthcare provider. Your healthcare provider may need to adjust your diabetes medication, exercise program, or diet.

Older adults may be more sensitive to side effects like dry mouth, dizziness, confusion, difficulty urinating, and QT prolongation (see above).

Aventyl should only be used during pregnancy if clearly necessary. Discuss risks and benefits with your healthcare provider before using it if you are pregnant or planning to become pregnant.

Aventyl passes into breast milk and may harm a nursing infant. Consult your healthcare provider before breastfeeding.
