Janumet XR is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when treatment with both sitagliptin and metformin extended-release is appropriate. It should not be used in patients with type 1 diabetes mellitus or for the treatment of diabetic ketoacidosis.

This medication contains two prescription diabetes medicines, sitagliptin, and extended-release metformin. Sitagliptin works by regulating the levels of insulin the body produces after eating, while metformin works by decreasing glucose (sugar) production in the liver and decreasing the absorption of glucose by the intestines.

Do’s

  • Read the Medication Guide provided by your pharmacist before you start using Janumet XR and each time you get a refill. If you have any questions, ask your healthcare provider.
  • Take this medication by mouth as directed by your healthcare provider, usually twice a day with meals.
  • Drink plenty of fluids while taking this medication unless otherwise directed by your healthcare provider.
  • If you have trouble swallowing the tablet whole, tell your healthcare provider.
  • The dosage is based on your medical condition, response to treatment, and other medications you may be taking. Be sure to tell your healthcare provider about all the products you use (including prescription drugs, nonprescription drugs, and herbal products).
  • To reduce your risk of side effects (such as upset stomach), your healthcare provider may direct you to start this medication at a low dose and gradually increase your dose. Follow their instructions carefully.
  • Take this medication regularly to get the most benefit from it. To help you remember, take it at the same time each day. 
  • Carefully follow the medication treatment plan, meal plan, and exercise program your healthcare provider has recommended.
  • Tell your healthcare provider if your condition does not improve or if it worsens (your blood sugar is too high or too low).

Additionally, weight loss is a possible side effect when taking Janumet XR, although this medications are not approved for weight loss.

Metformin, one of the components of Janumet XR, can lead to a rare but serious condition called lactic acidosis, which can be life-threatening. Be vigilant for symptoms of lactic acidosis, such as muscle pain, difficulty breathing, stomach discomfort, and unusual tiredness, and seek immediate medical attention if these symptoms occur.

Janumet XR is contraindicated in patients with an estimated glomerular filtration rate (eGFR) below 30 mL/min/1.73 m². Discontinuation of Janumet XR is recommended if the patient's eGFR later falls below this threshold. Additionally, caution should be exercised when considering the use of Janumet XR in patients with renal impairment.

Discontinuation of Janumet XR is advised at the time of, or prior to, an iodinated contrast imaging procedure in patients with specific eGFR levels. Re-evaluation of renal function is recommended 48 hours after the imaging procedure, with the potential for restarting Janumet XR if renal function is stable.

Janumet XR has not been studied in patients with a history of pancreatitis. It is unknown whether patients with a history of pancreatitis are at an increased risk for the development of pancreatitis while using Janumet XR. Therefore, caution is warranted in such cases.

The safety of taking Janumet XR during pregnancy and breastfeeding is not fully established. Discuss the risks and benefits of Janumet XR with a healthcare provider if pregnancy or breastfeeding is a consideration.
