Topamax Sprinkle (Topiramate)

Do’s

• Before starting Topamax Sprinkle, read the Medication Guide provided by your pharmacist. If available, also review the Patient Information Leaflet. If you have any questions, ask your healthcare provider.
• Take Topamax Sprinkle by mouth with or without food as directed by your healthcare provider. Usually, it is taken twice daily.
• Topamax Sprinkle capsule can be swallowed whole or opened to sprinkle the contents on a teaspoonful of soft food, such as applesauce or yogurt. If you choose to sprinkle the contents, swallow the drug/food mixture right away without chewing.
• After swallowing the mixture, drink some liquid to ensure all of the drug is swallowed.
• The dosage of Topamax Sprinkle is based on your medical condition and response to treatment. For children, the dosage may also be based on weight. Your healthcare provider will gradually increase your dose to reduce the risk of side effects. In some cases, treatment may start with a once-daily dose at bedtime, which is then slowly increased to twice a day. It may take several weeks or months to reach the optimal dose for you and to experience the full benefits of the medication.
• Take Topamax Sprinkle regularly at the same times each day to get the most benefit from it. This can help you remember to take it. 
• To prevent kidney stones from forming, drink plenty of liquids while taking Topamax Sprinkle, unless your healthcare provider advises otherwise.
• If your condition persists or worsens while taking Topamax Sprinkle, inform your healthcare provider. 

Don’ts

• Do not prepare the mixture in advance for later use. 
• Do not stop taking the medication without consulting your healthcare provider, as some conditions may worsen if the drug is suddenly stopped. If your doctor determines that you need to discontinue the medication, they will likely gradually decrease your dose.

Common Side Effects:

• Tingling or numbness in the hands or feet
• Dizziness or drowsiness
• Fatigue or tiredness
• Loss of appetite
• Nausea or vomiting
• Diarrhea
• Weight loss

Less Common but Potentially Serious Side Effects:

• Mood changes, such as depression or suicidal thoughts
• Vision problems or eye pain
• Rapid breathing or irregular heartbeat
• Bone pain or fractures
• Metabolic acidosis 

Seek medical attention if you experience any severe or persistent side effects while taking Topamax Sprinkle.

Topamax Sprinkle has been associated with cases of acute myopia (sudden nearsightedness) and secondary angle closure glaucoma. These conditions can cause ocular pain, blurred vision, and increased intraocular pressure. If you experience any sudden vision changes or eye pain, seek medical help immediately.

Overdose of Topamax Sprinkle can lead to severe metabolic acidosis, a condition characterized by an imbalance of acid and base in the body. In case of overdose, the medication should be discontinued, and general supportive treatment should be provided until clinical toxicity is resolved.

Topamax Sprinkle Capsules should be stored in tightly closed containers at or below 25°C (77°F) and protected from moisture.

Topamax Sprinkle may interact with other medications. Inform your healthcare provider about all the medications you are taking, including prescription, over-the-counter, and herbal products, to avoid potential interactions.

Topamax Sprinkle is not recommended for the treatment or prevention of migraines in children due to insufficient data on safety and efficacy.

When initiating Topamax Sprinkle monotherapy in newly diagnosed epileptic patients or when converting to monotherapy, the dosage should be initiated at a low dose and adjusted gradually. The withdrawal of concomitant antiepileptic agents should be done gradually to consider the effects of seizure control.
