Acid Reflux Disease

Acid reflux or Gastroesophageal reflux (GERD) disease occurs when the acid rich liquid content of the stomach regurgitates into the esophagus. This acid-rich liquid can inflame and damage the lining of your esophagus. The refluxed liquid may also contain bile that that has backed-up into the stomach from the small intestine (duodenum).

The stomach acid is the most harmful component of the refluxed liquid. Although bile may damage your esophagus, the extent of the damage is not as completely understood as the damaging effects of stomach acid.

Unfortunately, acid reflux is a chronic condition. There is no acid reflux cure. Even after you receive treatment for acid reflux disease and your esophagus heals, the symptoms of acids reflux will return, and the damage will reoccur.

Acid reflux symptoms

There is not a clear-cut cause of acid reflux. The causes of acid reflux can vary from person to person. Some people simply produce a higher amount of stomach acid, but in most cases the following factors contribute to acid reflux in patients:
  • Obesity - Excess weight puts extra pressure on your stomach and diaphragm, the large muscle that separates your chest and abdomen, forcing open the lower esophageal sphincter and allowing stomach acids to back up into your esophagus. Eating very large meals or meals high in fat may cause similar effects.
  • Pregnancy - Pregnancy results in greater pressure on the stomach and a higher production of the hormone progesterone. This hormone relaxes many of your muscles, including the lower esophageal sphincter.
  • Asthma - Doctors aren't certain of the exact relationship between asthma and heartburn. It may be that coughing and difficulty exhaling lead to pressure changes in your chest and abdomen, triggering regurgitation of stomach acid into your esophagus. Some asthma medications that widen (dilate) airways may also relax the lower esophageal sphincter and allow reflux. Or it's possible that the acid reflux that causes heartburn may worsen asthma symptoms. For example, you may inhale small amounts of the digestive juices from your esophagus and pharynx, damaging lung airways.
  • Diabetes - One of the many complications of diabetes is gastroparesis, an uncommon disorder in which your stomach takes too long to empty. Left in your stomach too long, stomach contents can regurgitate into your esophagus and cause heartburn.
  • Peptic ulcer - An open sore or scar near the valve (pylorus) in the stomach that controls the flow of food into the small intestine can keep this valve from working properly or can obstruct the release of food. Food doesn't empty from your stomach as fast as it should, causing stomach acid to build up and back up into your esophagus.
  • Delayed stomach emptying - The most common time for acid reflux is during the day, after you eat. Studies show reflux commonly occurs at these times as a result of LES relaxations caused by distention of the stomach with food. In addition to diabetes or an ulcer, abnormal nerve or muscle functions can delay emptying of your stomach, causing acid backup into the esophagus.
  • Connective tissue disorders - Diseases such as scleroderma that cause muscular tissue to thicken and swell can keep digestive muscles from relaxing and contracting as they should, allowing acid reflux.
  • Zollinger-Ellison syndrome - One of the complications of this rare disorder is that your stomach produces extremely high amounts of acid, increasing the risk of acid reflux.
  • Lower esophageal sphincter (LES) - The LES is a specialized ring of muscle that surrounds the end of the esophagus joined to the stomach. Your LES opens when food or saliva is swallowed, but closes to prevent reflux. Acid reflux occurs when your LES abnormally relaxes or contracts weakly. Extended relaxation or weak contractions allow more acid to travel back up the esophagus.
  • Esophageal contractions - Swallowing is a key factor in preventing acid reflux. When you swallow, you contract the esophageal muscles. This contraction pushes food, saliva and acid back into your stomach. In certain patients, this contraction is not strong enough to push the acid back into the stomach.
  • Hiatal hernia - Hiatal hernias are another contributing factor to acid reflux, although doctors and researchers are not sure how they contribute. Studies show that a majority of patients who suffer from acid reflux also have hiatal hernias.

    Studies also indicate that people with hiatal hernias don't always suffer from acid reflux disease. A hiatal hernia occurs when a small part of your upper stomach that attaches to the esophagus is pushed through the diaphragm. This forces the lower esophageal sphincter (LES) to lie in the chest. As a result, your LES is no longer level with your diaphragm. Normally, the pressure exerted by your diaphragm and LES prevents reflux, but after the LES is pushed into the chest, the additive pressure of is decreased.

    Hiatal hernias may also contribute to your acid reflux disease because when a hiatal hernia is present, a hernial sac is also present. This hernail sac is separated from your esophagus by the LES and the diaphragm. In certain cases, the hernail sac traps acid coming from your stomach, which means it is easier for the acid to reflux whenever your LES relaxes.

Symptoms of acid reflux oesophagitis

  • A painful or burning sensation in the upper abdomen or chest, sometimes radiating to the back (heartburn).
  • Difficulties breathing and suffering from hoarseness because the refluxed fluid irritates the larynx and respiratory tract.
  • Excess belching is common.

When does acid reflux occur?

  • After eating a large or fatty meal.
  • After drinking alcohol.
  • Lying down, bending over or bending and lifting.
  • After smoking.
  • After eating chocolate, caffeine, onions, spicy foods, mint and some medications.
  • Lying down soon after eating.
  • After taking tranquilizers, such as benzodiazepines including diazepam (Valium) and alprazolam (Xanax).
  • After taking Theophylline (Elixophyllin), an asthma medication. In some cases, acid reflux symptoms may have no apparent cause.

Alendronate for osteoporosis

Most people assume that bones can only break under extreme conditions. Bones are actually comprised of living tissue that is constantly replaced by new, stronger tissue. If the regeneration cycle is altered, your body will remove more tissue than it creates. This is known as osteoporosis.

There are no symptoms for osteoporosis. You can't feel your bones weakening, so osteoporosis often goes undiagnosed until you actually break a bone.

Alendronate

UTI
Urinary tract infections, or UTIs, are common and affect millions of people each year. Although both men and women can develop UTIs, women suffer from them more frequently than men.

Urinary tract infections (UTIs) are treated with antibiotics. These drugs kill the bacteria that causes the infection. For uncomplicated UTIs (UTIs that develop in normally healthy people), antibiotics are normally prescribed for 3-7 days, depending on how often UTIs reoccur.

Prostatitis

  1. Acute bacterial prostatitis is the least common of the four types but also the easiest to diagnose and treat effectively. Men with this disease often have chills, fever, pain in the lower back and genital area, urinary frequency and urgency often at night, burning or painful urination, body aches, and a demonstrable infection of the urinary tract as evidenced by white blood cells and bacteria in the urine.   2. Chronic bacterial prostatitis, also relatively uncommon, is acute prostatitis associated with an underlying defect in the prostate, which becomes a focal point for bacterial persistence in the urinary tract. Effective treatment usually requires identifying and removing the defect and then treating the infection with antibiotics. However, antibiotics often do not cure this condition.

Ciprofloxacin


Ciprofloxacin is a prescription medication that blocks bacterial DNA replication by binding itself to an enzyme called DNA gyrase. The gyrase enzyme prevents the untwisting required to replicate one DNA double helix into two, and is commonly prescribed to fight UTIs and prostatis.

  • Drink plenty of fluids (8 to 10 cups a day) such as water, sports drinks, herbal teas, fruit drinks, or ginger ale. Even clear soups - yes, chicken soup. Fluids help break up congestion, prevent dehydration and keep your throat moist.
  • Inhaled steam can ease congestion too. Create steam with a humidifier, or steam up the bathroom by running a hot shower. Or create a mini-steamer by boiling water on the stove and placing the pot on a table. Drape a towel over your head and the water (careful!) and inhale the steam. The effect is enhanced if you add a half teaspoon of Vicks VapoRub, 1-2 drops of eucalyptus oil or a few slices of ginger to the boiling water.

For nasal congestion:

  • Relieve clogged nasal and sinus passages caused by excessive mucus with either decongestant pills or liquid medication or with a nasal spray. Oral decongestants you can buy at your drug store contain pseudoephedrine (with brand names such as Sudafed or Triaminic). This ingredient can cause an increase in heart rate and blood pressure and may keep you awake because it's a stimulant. Anyone with high blood pressure, heart disease, diabetes, thyroid disease, or glaucoma (high pressure of the eye) should not use these decongestants. Depending on state law, you may have to ask and sign for products containing pseudoephedrine because it is used in making illegal street drugs.

  • Nasal spray decongestants do the same thing and have the advantage of going directly to the problem area - your nose. The active ingredient is oxymetazoline found in brand names such as Afrin, Dristan nasal spray, Neo-Synephrine, and Vicks Sinex. Do not use these sprays longer than 3 days or you will risk getting a rebound effect - your nose will become stuffy once again. These are best taken following a hot shower and lots of nose blowing to clear out the mucus as much as possible. Nasal sprays are a good alternative to pseudoephedrine at bedtime because they won't interfere with sleep.
For fever and pain, body aches and tiredness:

  • Rest in bed. And if you're home in bed, you're not at work or around others and spreading your germs.

  • Over-the-counter medications such as acetaminophen (Tylenol) or ibuprofen (Advil or Motrin) can help decrease fever and ease sore throat pain and body aches. Never give a child aspirin or medications containing aspirin due to the risk of Reye Syndrome.
For cough:

  • For a dry hacking cough, you may choose a medication that contains a cough suppressant - to block the cough reflex and keep the cough from waking you at night. Look for over-the-counter medications that contain dextromethorphan, like Benylin DMD.

  • For a cough that produces excessive mucus, or phlegm, you may want to use an expectorant to loosen secretions so you can cough them into a tissue. A cough is a reflex. When your airway passages are irritated, you cough. Guaifenesin is the most common active ingredient in over-the-counter expectorants (for example, Benylin DMDE). Help them work by drinking plenty of fluids.
For sore throat:

  • A warm salt-water gargle can relieve a scratchy throat. Dissolve about a half teaspoon of salt in a glass of warm water.

  • Lozenges, mouthwashes, and sprays that contain a numbing ingredient such as benzocaine, phenol, or menthol can ease the pain. Some people think lozenges containing zinc relieve cold symptoms, but the benefits are not proven, and zinc can cause stomach upset and has an unpleasant taste.
When to contact the doctor: Call your doctor if your symptoms of cold or flu last more than 10 days, if you're not getting better, if your fever goes above 102 degrees F, or if you feel shortness of breath, or other worrisome symptoms.
